If you have private health insurance please bring your members card.  If you have any relevant information, such as x-rays or a letter from your Medical Practitioner, please bring these with you.

Any images taken in Geelong at Lake Imaging, GMI , BMI, Radius or Epworth Imaging we will have internet access to those (with your permission of course). 

Those taken elsewhere you will need to bring with you via disc or hard copy.
